Head Start Information—Transition Coordinators and Others

Transition Coordinators—From Planning To Evaluation

If you are a Transition Coordinator, you will be responsible for building and maintaining the system that will support the transition of children as they prepare for one of the most important transition experiences of their life—the transition to school. Your Head Start program will provide you with a job description that will be individualized for the program but should generally include activities in the following four areas:

Planning—You will oversee the development of transition plans that include timelines, goals, action steps, and outcomes. You may also assist the program as it develops other program plans such as strategic plans or service area plans to make sure that the topic of transition has been appropriately included.

Implementing—You will ensure that the program’s transition plan is used consistently by all staff to help the program remain on task with important transition timelines and to support program compliance with all Head Start regulations related to transition. You should also ensure that staff members understand the importance of creating transition plans for individual children and families based upon the program’s overall transition process.

Training—You may be responsible for providing transition training—including details of the transition to Kindergarten—for parents, staff, community members, or local schools.

EvaluatingYou should incorporate methods for evaluating the effectiveness of the program’s transition process and align these methods with other major initiatives such as collecting outcomes data from schools.
