Eligibility (Determining/Confirming Eligibility for McKinney-Vento) NCHE Local Liaison Toolkit Appendix D: Enrollment Tools Appendix D of the NCHE Local Liaison Toolkit provides sample forms a school district may use in the school enrollment of a child or youth experiencing homelessness. Sample forms include: Student Residency Form, Determining Feasibility of School Placement, Affidavit for Missing Enrollment Documentation, Caregiver Authorization Form, and Written Notification of Enrollment Decision. The Council of American Building Officials (CABO): One and Two Family Dwelling Code CABO codes are recommended by HUD as a guidline for establishing state and local building codes. State and local building codes may be used to determine whether a student's residence would be considered substandard, thereby qualifying the student for McKinney-Vento services.District School Board of Pasco County: Student/Family Domicile Questionnaire This one-page questionnaire is used by the District School Board of Pasco County to determine eligibility for services under Title I, Part A, Title I, Part C-Migrant, or Title X, Part C-McKinney-Vento. Other districts may use this form as a guide for developing their own domicile questionnaires.Electronic Code of Federal Regulations: Title 24, Part 200, Subpart 2 - Minimum Property Standards These regulations discuss HUD's guidelines for the establishment of state or local building codes. State building codes may be used as standards by which to evaluate whether a student's residence would be considered substandard, thereby qualifying the student for McKinney-Vento services.Louisiana Student Residency Questionnaire This one-page questionnaire, developed by the Louisiana Education of Homeless Children and Youth Program (EHCY), may be used to determine homelessness upon school enrollment. Other states may use this form as a guide for developing their own residency questionnaires.Texas Student Residency Questionnaire This one-page questionnaire, developed by the Texas Homeless Education Office (THEO), may be used to determine homelessness upon school enrollment. Other states may use this form as a guide for developing their own residency questionnaires.Wenatchee, WA: Housing Codes and Definition of Substandard Housing The McKinney-Vento Homeless Assistance Act's definition of "homeless" includes children and youth living in "substandard housing". Since the federal law does not outline a specific definition of "substandard housing", states and communities are encouraged to use state and/or local building codes and norms when determining how it will define "substandard housing".
