District School Board of Pasco County: Student/Family Domicile Questionnaire
This one-page questionnaire is used by the District School Board of Pasco County to determine eligibility for services under Title I, Part A, Title I, Part C-Migrant, or Title X, Part C-McKinney-Vento. Other districts may use this form as a guide for developing their own domicile questionnaires.

San Antonio Independent School District Transportation Brochure for Parents
This brochure, created by the Transitions Program of the San Antonio Independent School District (SAISD), explains the SAISD's transportation policy to parents of students experiencing homelessness. This bilingual brochure (English and Spanish/inglés y español) can be adapted for use in other school districts.

Spokane Public Schools District Transportation Agreement
This agreement, developed by HEART (Homeless Education & Resource Team) at Spokane Public Schools, outlines the district's transportation policy and guidelines for students experiencing homelessness. It requires the signature of the homeless student's parent or guardian or of the homeless unaccompanied youth.
